HLTH 1540 - Community Health Worker II


4.5 Credits

This course will prepare students to work as trained health educators with community members who may have difficulty understanding providers due to cultural or language barriers. Students will explore working with under served communities, reducing health disparities, enhancing provider communication, and improving health outcomes and overall quality measures.

Course Objectives
  1. Students will be able to explain care management and design a client-based plan focused on strengths and needs of the person.
  2. Students will be able to identify and respond to challenges related to home visits and analyze safety issues.
  3. Students will be able to design and conduct an assessment of the home environment while maintaining ethical considerations of the client.
  4. Students will be able to clearly articulate in written form the outreach services needed and provided to clients and explain the importance of doing so through an outreach plan.
  5. Students will be able to explore community resources for mental health services and analyze when appropriate referrals should be made for clients.
