HMSV 1140 - Assessment, Case Planning, and Management 4.5 Credits
Students learn the process of collecting pertinent data about client or client systems and their environment and appraising the data as a basis for making decisions regarding diagnosis, treatment, and/or referral of chemical dependency clients. Instruction on coordinating and prioritizing client treatment goals and working with other services, agencies, and resources to achieve those treatment goals is included. This course also includes practice in assessing and managing a case. This includes the development of sample case records and utilization of written client records to guide and monitor services with emphasis on the development of the social history and intake, initial assessment, case reviews and consultation, individual treatment plan with measurable goals and objectives, documentation of progress, on-going assessment, and discharge planning including appropriate referrals. Confidentiality of client information and records as defined in 42 CFR Part 2 is addressed. The strengths and weaknesses of various levels of care and the selection of an appropriate level for clients is studied. Basic information on two or more objective screening instruments for alcohol/drug disorders, such as the Michigan Alcoholism Screening Test (MAST), Substance Abuse Subtle Screening Inventory (SASSI), Addiction Severity Index (ASI), Mortimer-Filkins, and others are studied.

Course Objectives
- State why documenting and record keeping is necessary in chemical dependency counseling.
- Differentiate and describe the difference between evaluative and descriptive statements.
- Prepare records using a variety of recording methods.
- Define and practice confidentiality and its critical importance for the agency and its clients.
- Prepare and enter case notes in a case file.
- Compile a case file of case notes, activities, and proper placement of documents.
- Describe responsibilities of a case manager and the factors and practices of appropriate referrals.
- Identify and practice assessment and intervention strategies.
